Abstract

This study investigated the knowledge, attitudes, and practices (KAP) concerning medication error detection and reporting among pharmacy technician students at Archbishop Charles Herry College of Health Science and Technology, Nkpor. A cross-sectional survey research design was adopted, utilizing a structured questionnaire to collect data from a sample of 333 respondents. The data were analyzed using SPSS version 27, employing descriptive statistics to summarize the data and t-tests to test the stated hypotheses. The findings revealed a significant positive relationship between students’ knowledge levels and their ability to detect medication errors. Additionally, students’ attitudes toward medication error detection and reporting significantly influenced their willingness to report errors. The study concluded that enhancing students’ knowledge and fostering positive attitudes are crucial for improving medication error detection and reporting practices. Recommendations include implementing comprehensive training programs, creating supportive reporting environments, promoting interprofessional collaboration, utilizing technology to support error detection and reporting, and conducting regular audits and feedback mechanisms. These measures aim to equip pharmacy technician students with the necessary skills and attitudes to contribute to safer medication practices in clinical settings.

 

 

 

 

 

 

 

CHAPTER ONE
INTRODUCTION

1.1       Background to the Study

The global healthcare industry is continuously evolving with an overarching goal to improve patient outcomes through the provision of safe, effective, and high-quality care. Despite significant technological and clinical advancements, medication errors persist as a major threat to patient safety, particularly in resource-constrained settings (Goyal et al., 2014). Medication errors—defined as preventable events that may lead to inappropriate medication use or harm to a patient while the medication is under the control of a healthcare provider, patient, or consumer—can occur at various points in the medication use process, including prescribing, dispensing, administration, and monitoring (Qaseem et al., 2017).

Pharmacy technicians are essential healthcare personnel whose roles encompass the accurate preparation and dispensing of medications, inventory management, and often serving as a communication link between pharmacists and other healthcare workers. Their position makes them uniquely placed to identify potential errors before they reach the patient. Therefore, it is vital that pharmacy technician students, during their training, are adequately prepared to recognize and respond to medication-related safety concerns (Clark et al., 2021). However, empirical evidence suggests that in many developing countries, including Nigeria, this group remains undertrained in error detection due to insufficient clinical exposure, poor supervision, and lack of structured modules focusing on patient safety (Carè et al., 2021).

In Nigeria, the increasing reliance on pharmacy technicians—especially in primary and secondary healthcare facilities—has led to a pressing need for a better understanding of their training and competencies in medication safety. As frontline staff involved in medication preparation and distribution, they are often the last checkpoint before medications reach patients. Any error left unchecked can have devastating effects, including treatment failures, increased morbidity, or even death. The World Health Organization (WHO) has identified medication safety as a global health priority through its “Medication Without Harm” initiative, which aims to reduce severe, avoidable medication-related harm by 50% over five years (Elgaddal & Weeks, 2024).

Medication errors are not only costly but also erode public confidence in healthcare systems. The WHO initiative underscores the critical role of healthcare professionals, including pharmacy technicians, in advancing medication safety. A significant component of achieving this involves equipping students in health training institutions with the knowledge, attitudes, and practical skills necessary for the early identification and reporting of such errors (Hilton et al., 2017).

At the educational level, a gap often exists between classroom learning and real-world clinical practice. Pharmacy technician students may learn about medication safety theoretically but lack opportunities to practice these skills in realistic settings. According to Dydyk and Conermann (2024), this disconnect between training and practical experience may contribute to a lower rate of error recognition and prevention in clinical environments. A contributing factor may also be a lack of reinforcement mechanisms, such as role-modeling by supervisors or inter-professional team-based simulations that encourage error detection behavior.

Furthermore, research into mindfulness and attention regulation has revealed that skills such as sustained attention, heightened awareness, and presence of mind—often enhanced through meditation and mindfulness practices—can potentially improve clinical decision-making and error detection capabilities (Tang et al., 2023; Black & Slavich, 2016). Studies by Zeidan and Vago (2016) suggest that the practice of mindfulness meditation can improve cognitive function, reduce stress, and increase attentional control—all of which are crucial for healthcare providers in high-stakes environments. While meditation is not a core part of pharmacy education, its cognitive and psychological benefits provide a useful lens through which training programs might be enhanced to foster better situational awareness and reduce preventable errors.

In the Nigerian context, few studies have directly addressed the knowledge, attitude, and practice (KAP) of pharmacy technician students concerning medication error detection. However, similar studies in related fields suggest that these students may possess varying levels of awareness and differing attitudes based on exposure, instructional quality, and institutional commitment to safety culture (Wasil et al., 2020; Shepardson et al., 2023). For instance, Sharma (2023) emphasized the role of structured training in cultivating a proactive attitude toward healthcare safety, while Hearn, Finlay, and Sheffield (2021) noted that barriers to awareness are often rooted in institutional neglect of experiential learning.

Archbishop Charles Heerey College of Health Science and Technology, Nkpor, plays a pivotal role in training mid-level healthcare professionals, including pharmacy technicians. Located in Anambra State, the institution serves a large catchment area and contributes significantly to the region’s health workforce. However, little is known about how well its students are prepared in the realm of medication safety, particularly concerning detection and reporting of errors. Understanding the baseline knowledge, prevailing attitudes, and actual practices of students at the college is a foundational step toward developing effective interventions.

Moreover, cognitive-behavioral theories, such as the Transtheoretical Model of Behavior Change, emphasize that knowledge alone is insufficient to prompt behavior change. A combination of internal motivation, perceived susceptibility, and skill development is required for students to take proactive actions (Prochaska et al., 2015). In line with this, mindfulness centers such as the Benson-Henry Institute for Mind Body Medicine and the UCSF Osher Center for Integrative Medicine have explored ways in which awareness training can complement traditional healthcare education (Benson-Henry Institute, 2024; Osher Center, 2024). These models present an opportunity for integration into the existing pharmacy technician curriculum to reinforce patient safety protocols.

Ultimately, this study seeks to fill an existing research gap by focusing on the KAP levels of pharmacy technician students at Archbishop Charles Heerey College concerning medication error detection. By identifying specific knowledge gaps, evaluating students’ attitudes toward error reporting, and assessing actual safety practices, this study aims to inform policy development, curriculum revision, and practice improvement in the institution. This is consistent with the broader goal of minimizing preventable harm in healthcare settings, as advocated by both national regulatory agencies and global health organizations (Deniz et al., 2021).

Addressing this issue is not only academically relevant but also vital for the protection of public health. As Nigeria works toward strengthening its healthcare delivery systems, investing in the safety competencies of all cadres of health workers—including pharmacy technicians—will be essential. Institutions such as Archbishop Charles Heerey College have the responsibility to lead this change by incorporating evidence-based teaching practices, promoting experiential learning, and fostering an institutional culture that values error prevention, transparency, and continuous quality improvement.

1.2       Statement of the Problem

Chronic pain and psychological distress continue to pose significant challenges to individual well-being and public health, especially in contexts where pharmacological interventions are limited or insufficient. Despite growing interest in non-pharmacologic approaches such as mindfulness meditation, yoga, and other integrative practices (Dydyk & Conermann, 2024; Tang, Hölzel, & Posner, 2023; Sharma, 2023), the translation of these practices into routine clinical settings remains limited by methodological, systemic, and psychosocial barriers. Research has established that mindfulness and meditation-based therapies can reduce stress and pain intensity (Zeidan & Vago, 2016; Chiesa & Serretti, 2011), enhance immune functioning (Black & Slavich, 2016), and support mental health outcomes (Hilton et al., 2017). However, much of this literature is either constrained to Western populations or fails to consider variability in individual adherence, socio-demographic factors, and cultural perceptions influencing practice uptake.

Moreover, there is a growing disconnect between evidence-based meditation content and the actual offerings in popular self-help platforms and applications (Wasil et al., 2020). While some institutions such as the Benson-Henry Institute and the Osher Center at UCSF promote structured integrative practices (Benson-Henry Institute, 2024; Osher Center, 2024), few empirical studies explore how such models can be scaled to underserved or high-need populations. Similarly, while stakeholders broadly agree on the need for integrative health regulation (Carè, Steel, & Wardle, 2021), implementation science methodologies remain underutilized in this field (Clark et al., 2021).

A key research gap lies in the underexplored barriers to meditation uptake among specific populations, such as caregivers, low-income communities, and people with physical disabilities (Williams et al., 2012; Hearn, Finlay, & Sheffield, 2021). Factors such as age, gender, and patient-physician relationships have been identified as significant (Deniz et al., 2021; Shepardson et al., 2023), but nuanced understanding remains lacking, particularly in non-Western or multicultural settings. Additionally, few studies use validated inventories to measure long-term adherence and outcomes of meditation practices (Williams et al., 2011).

Therefore, this study seeks to fill these gaps by investigating the multi-level barriers and facilitators to mindfulness and integrative practices, focusing on diverse socio-demographic settings. It aims to extend current knowledge by applying mixed-method approaches to explore how psychosocial, cultural, and technological factors influence the adoption and sustainability of these interventions. Ultimately, the study intends to contribute actionable insights for policy, clinical integration, and personalized health strategies that support holistic well-being.

1.3       Objectives of the Study
The main objective of this study is to assess the knowledge, attitude, and practice of detecting medication errors among pharmacy technician students of Archbishop Charles Heerey College of Health Science and Technology, Nkpor. Specifically, the study seeks to:

  1. Evaluate the level of knowledge that pharmacy technician students have concerning medication error detection.
  2. Assess the attitudes of pharmacy technician students toward the detection and reporting of medication errors.
  3. Examine the practical steps taken by pharmacy technician students in identifying and managing medication errors during clinical training.

1.4       Research Questions
In line with the objectives of the study, the following research questions will guide the investigation:

  1. What is the level of knowledge among pharmacy technician students regarding medication error detection?
  2. What attitudes do pharmacy technician students hold towards the detection and reporting of medication errors?
  3. What practices are pharmacy technician students adopting in detecting and managing medication errors?

1.5       Research Hypotheses
To provide a framework for statistical analysis and interpretation, the study will test the following hypotheses:

  1. H₀₁: There is no significant relationship between the level of knowledge and the ability to detect medication errors among pharmacy technician students.
  2. H₀₂: There is no significant association between students’ attitudes and their willingness to report medication errors.
  3. H₀₃: There is no significant relationship between knowledge level and the practical detection of medication errors during clinical practice.

1.6       Significance of the Study
This study is significant for several reasons. Firstly, it contributes to the body of academic literature on healthcare safety, particularly the role of pharmacy technician education in minimizing medication errors. By evaluating the knowledge, attitudes, and practices of students, the research provides insights into the effectiveness of current training curricula and identifies gaps that need to be addressed.

Secondly, the findings will assist academic administrators and educators at Archbishop Charles Heerey College of Health Science and Technology, Nkpor, in developing more targeted teaching strategies, simulation-based learning, and clinical exposure to enhance students’ competencies in medication safety. The results may also prompt curriculum reform to integrate standardized modules on pharmacovigilance and error reporting.

Thirdly, the study has practical implications for hospital and community pharmacy settings where pharmacy technicians are deployed. Better-trained students translate to fewer medication errors, improved patient safety, and more efficient healthcare delivery. Regulatory bodies and professional associations can also use the findings to establish minimum competency standards for pharmacy technician training across Nigeria.

Lastly, this research will raise awareness among students themselves, encouraging them to take a more proactive approach to medication safety and to view error detection not as a punitive issue but as a vital component of patient-centered care.
