Abstract

The study focused on investigating the prevalence of malaria among pregnant women attending primary healthcare centres in the Abbi community, as well as the effectiveness of existing prevention and control strategies. A quantitative survey research design was adopted, and data were collected using a structured questionnaire from a sample of 120 respondents. SPSS27 was utilized to present and analyze the collected data, with t-tests employed to test the hypotheses formulated. Findings revealed a concerning prevalence of malaria among pregnant women in the study area, with a majority of respondents either strongly agreeing or agreeing with the statement. Additionally, the study identified various risk factors associated with malaria infection among pregnant women, indicating a need for targeted interventions. Moreover, while existing malaria prevention strategies were perceived to be effective by a significant proportion of respondents, there were still uncertainties and disagreements among some participants. In conclusion, the study highlights the importance of addressing malaria among pregnant women in the Abbi community through comprehensive prevention and control strategies. Based on the findings, it is recommended that healthcare providers enhance the accessibility and utilization of antenatal care services, strengthen community health education programs, and continually monitor and evaluate the effectiveness of existing malaria prevention measures. Overall, the study contributes valuable insights into the malaria burden among pregnant women in the Abbi community and provides a foundation for future research and interventions aimed at improving maternal and child health outcomes in the region.

 

 

 

 

 

 

 

 

CHAPTER ONE

INTRODUCTION

1.1       Background to the Study

Malaria remains a significant global health concern, particularly in sub-Saharan Africa where it poses a considerable burden on public health systems. The World Health Organization (WHO) reports that malaria continues to be a leading cause of morbidity and mortality in many endemic regions, with pregnant women being among the most vulnerable populations (WHO, 2017). This vulnerability stems from physiological changes that occur during pregnancy, which can make women more susceptible to infection and increase the risk of severe complications if infected (Iriemenam et al., 2021).

In endemic regions like the Abbi community in Ndokwa West Local Government, malaria presents a substantial threat to the health and well-being of pregnant women and their unborn children. According to the Nigeria Malaria Indicator Survey 2015 conducted by the National Malaria Elimination Programme (NMEP), malaria remains prevalent in Nigeria, including in rural communities like Abbi (NMEP, 2022). This poses significant challenges for pregnant women seeking antenatal care and delivery services, as malaria can lead to adverse outcomes such as low birth weight, preterm birth, and maternal mortality (Akaba et al., 2021; Obol et al., 2021).

Despite various interventions implemented by government and non-governmental organizations, malaria continues to be a significant cause of morbidity and mortality among pregnant women in Abbi community. Studies have shown that knowledge and utilization of malaria preventive measures among pregnant women in Nigeria are often suboptimal, leading to increased risk of infection (Akaba et al., 2021). Furthermore, misconceptions about malaria transmission and prevention among pregnant women in post-conflict internally displaced persons’ camps contribute to the persistence of the disease in vulnerable populations (Obol et al., 2021).

Efforts to combat malaria among pregnant women in Abbi community must address not only access to preventive measures such as insecticide-treated bed nets and intermittent preventive treatment but also the knowledge gaps and misconceptions that hinder effective prevention and control. Community-based education and outreach programs can play a crucial role in improving awareness and promoting appropriate preventive behaviors (Amusan et al., 2017; Singh et al., 2022).

Additionally, interventions aimed at addressing broader social determinants of health, such as poverty and inadequate healthcare infrastructure, are essential for reducing the burden of malaria among pregnant women in Abbi community. The National Malaria Strategic Plan 2014–2020, developed by the Federal Ministry of Health and NMEP, provides a framework for coordinated action to control malaria in Nigeria, including strategies tailored to vulnerable populations like pregnant women (FMoH & NMEP, 2020).

1.2       Statement of Problem

The statement of the problem regarding malaria among pregnant women in the Abbi community of Ndokwa West Local Government encompasses several critical gaps that need to be addressed. Despite extensive efforts to combat malaria, pregnant women in this region continue to face significant risks of infection and related complications. One primary gap lies in the inadequate knowledge and utilization of malaria preventive measures among pregnant women. Studies have shown that pregnant women in Nigeria often lack awareness of effective preventive strategies, leading to increased susceptibility to malaria infection (Akaba et al., 2021). Addressing this knowledge gap is essential for improving the uptake of preventive measures and reducing the incidence of malaria among pregnant women in Abbi.

Furthermore, there is a lack of targeted interventions specifically tailored to the needs of pregnant women in the community. While broader malaria control programs exist, they may not adequately address the unique vulnerabilities and challenges faced by pregnant women. For example, misconceptions about malaria transmission and prevention among pregnant women in post-conflict internally displaced persons’ camps highlight the need for targeted education and outreach efforts (Obol et al., 2021). Developing and implementing interventions that specifically target pregnant women, such as antenatal education programs and access to preventive services, are crucial steps in addressing this gap.

Additionally, there are challenges related to access to healthcare services and resources for malaria prevention and treatment among pregnant women in Abbi. Limited access to antenatal care services, insecticide-treated bed nets, and intermittent preventive treatment can hinder efforts to prevent and manage malaria during pregnancy (National Malaria Elimination Programme [NMEP], 2022). Improving access to these essential resources and services, particularly in rural and underserved areas like Abbi, is critical for reducing the burden of malaria among pregnant women.

In essence, the gaps in knowledge, targeted interventions, and access to healthcare services pose significant challenges in addressing malaria among pregnant women in the Abbi community. Addressing these gaps is essential for improving maternal and child health outcomes and reducing the burden of malaria in this vulnerable population.

1.3       Objectives of the Study

The objectives of this study were to:

  1. Investigate the prevalence of malaria among pregnant women attending primary healthcare centres in Abbi community.
  2. Identify the risk factors associated with malaria infection among pregnant women in the study area.
  3. Assess the effectiveness of existing malaria prevention strategies in primary healthcare centres for pregnant women in Abbi community.

1.4       Research Questions

  1. What is the prevalence of malaria among pregnant women attending primary healthcare centres in the Abbi community?
  2. What are the main risk factors associated with malaria infection among pregnant women in the study area?
  3. How effective are existing malaria prevention strategies in primary healthcare centers for pregnant women in Abbi community?

1.5       Research hypotheses

  1. There is no significant association between socio-demographic factors and malaria prevalence among pregnant women in Abbi community.
  2. Access to antenatal care services is negatively correlated with the use of malaria prevention measures among pregnant women in the study area.
  3. Adequate knowledge about malaria transmission and prevention is not associated with lower malaria prevalence among pregnant women in primary healthcare centers.

1.6       Significance of the Study

Firstly, the findings of this study can contribute to a deeper understanding of the impact of malaria on pregnant women in Abbi. Pregnancy induces physiological changes that can increase a woman’s susceptibility to malaria infection and exacerbate the severity of its consequences. By examining the prevalence of malaria among pregnant women, as well as its associated complications and outcomes, this study can highlight the urgent need for targeted interventions to protect the health and well-being of expectant mothers in Abbi.

Moreover, the study can offer insights into the effectiveness of existing malaria prevention strategies in the Abbi community. Despite efforts to implement preventive measures such as insecticide-treated bed nets and intermittent preventive treatment during antenatal care visits, the burden of malaria among pregnant women persists. 1.7       Scope of the Study

This study focused on pregnant women attending primary healthcare centers in Abbi community, Ndokwa West Local Government. It assessed the prevalence of malaria, identified associated risk factors, and evaluated the effectiveness of existing prevention strategies. The study did not extend to other communities or healthcare settings outside the specified scope.

1.8       Operational Definition of Terms

Malaria: A parasitic disease transmitted through the bite of infected mosquitoes, characterized by symptoms such as fever, chills, and body aches.

Pregnant Women: Women who are expecting a child, from conception to delivery.

Primary Healthcare Centers: Basic healthcare facilities providing essential services, including antenatal care, immunizations, and treatment of common illnesses, at the community level.

Prevalence: The proportion of individuals in a population with a particular condition or disease at a specific point in time.

Risk Factors: Characteristics or behaviors associated with an increased likelihood of developing a disease or experiencing a health-related outcome.
