EFFECTS AND CAUSES OF TYPHIOD AND SALMONELLA IN WOMEN
Abstract
This study investigated the prevalence and factors contributing to Typhoid and Salmonella infections in Edo State, Nigeria, focusing on the impact of sanitation, hygiene practices, and healthcare access. A quantitative survey design was adopted, utilizing a structured questionnaire to gather data from residents across v3.1arious demographic groups within the state. A simple random sampling technique was employed to select 500 participants from both urban and rural areas. Data collection involved administering the questionnaires, which addressed respondents’ knowledge of the infections, their hygiene practices, healthcare access, and experiences with Typhoid and Salmonella. The data were analyzed using descriptive statistics, including frequency distributions and percentages, to provide insights into the current situation. Key findings revealed a high prevalence of both infections in the study area, with contaminated water and poor sanitation being identified as major risk factors. Additionally, there was limited knowledge of the importance of completing antibiotic courses, contributing to a rise in antibiotic resistance. Access to healthcare services varied significantly between urban and rural areas, with rural residents facing more challenges in accessing timely and adequate treatment. The study concluded that Typhoid and Salmonella infections remained a significant health threat in Edo State, exacerbated by poor sanitation, inadequate healthcare facilities, and low levels of public awareness. It was recommended that the state government invest in improving water sanitation infrastructure, increase public health education campaigns focusing on proper hygiene practices, and enhance healthcare access, especially in rural areas. Additionally, a focus on regulating antibiotic use and addressing antibiotic resistance was deemed essential to reducing the burden of these infections in the future.
CHAPTER ONE
1.0 INTRODUCTION
Typhoid and Salmonella infections, caused by Salmonella enterica serotype Typhi and other strains of Salmonella, remain significant public health concerns, particularly in low-resource settings where sanitation and healthcare are limited (Crump et al., 2023; Stanaway et al., 2023). These infections primarily spread through contaminated food and water and have severe consequences on health, including fever, gastrointestinal issues, and in some cases, long-term complications (Prasad et al., 2024). Women, particularly in socio-economically disadvantaged contexts, face unique challenges, such as the impact of pregnancy and menstruation, which may complicate diagnosis and treatment (Ajibola et al., 2024). Addressing these gender-specific barriers is essential to reducing health disparities (Haftu et al., 2024).
1.1 BACKGROUND OF STUDY
Typhoid fever and Salmonella infections are significant public health issues caused primarily by Salmonella bacteria. The bacteria responsible for typhoid fever is Salmonella enterica serotype Typhi, while other forms of Salmonella infections, including Salmonella gastroenteritis, are caused by other Salmonella strains. These infections spread primarily through the ingestion of food or water contaminated with these bacteria. Poor sanitation, inadequate access to clean water, and improper food handling are the main risk factors for these diseases, making them prevalent in regions with poor hygiene infrastructure, especially in low-income or developing countries (Crump et al., 2023; Stanaway et al., 2023).
Historically, typhoid fever was associated with urban areas suffering from poor sanitation. However, as access to clean water has become more limited in rural areas, the prevalence of typhoid fever has expanded to these regions as well (Antillón et al., 2023). In addition to typhoid fever, Salmonella gastroenteritis, which results from the consumption of contaminated food products such as undercooked poultry, eggs, or unpasteurized milk, remains a serious public health challenge (Buckle et al., 2012). Both typhoid fever and gastroenteritis can result in a variety of symptoms, including high fever, abdominal pain, diarrhea, vomiting, and dehydration. While these diseases are treatable with antibiotics, their persistence, especially in the face of rising antibiotic resistance, poses a significant challenge to global public health efforts (Crump et al., 2023).
In particular, women in many regions face a disproportionate burden from these infections due to both biological and socio-economic factors. Biologically, women are particularly vulnerable to the complications of typhoid fever and other Salmonella infections because of the impact these diseases can have on reproductive health. For instance, during pregnancy, infections such as typhoid fever can lead to complications, including miscarriage, preterm labor, and stillbirth, which increase the risk of maternal morbidity and mortality (Haftu et al., 2024). Women who contract these infections may also experience prolonged illness, which can interfere with their ability to care for their children and families, affecting both their physical and mental well-being (Yemata et al., 2021). The socio-economic context further exacerbates the risk faced by women, especially in areas where gender inequalities limit their access to adequate healthcare and resources.
One of the significant challenges in managing these infections in women is the difficulty in diagnosing and treating them in resource-poor settings. In many developing countries, healthcare systems are often under-resourced, with limited access to medical diagnostics and treatments. Additionally, there is a widespread lack of awareness regarding the signs and symptoms of typhoid and Salmonella infections, which delays medical intervention. This delay can lead to more severe complications and, in some cases, death (Ajibola et al., 2024). In rural areas, access to healthcare facilities may be limited, and even when available, the quality of care can be inadequate due to shortages of trained healthcare professionals and medical supplies (Admassu et al., 2019). Moreover, in many low-resource settings, there are frequent reports of misdiagnosis or underdiagnosis of these infections, as symptoms often overlap with other tropical diseases, such as malaria and dengue (Mawazo et al., 2019).
The issue of antibiotic resistance is another major concern that contributes to the persistence of Salmonella infections. Over time, Salmonella bacteria have developed resistance to commonly used antibiotics, such as ampicillin, chloramphenicol, and trimethoprim-sulfamethoxazole, making treatment more difficult and expensive (Stanaway et al., 2023). In many cases, patients are given ineffective treatments, leading to prolonged illness and the spread of resistant strains, further complicating the public health response. Antibiotic resistance, combined with the limited availability of second-line antibiotics in many resource-poor regions, continues to be a significant challenge in managing these infections (Adesegun et al., 2020).
Additionally, women’s healthcare needs are often overlooked in the management of infectious diseases such as typhoid fever and Salmonella infections. Women, particularly in certain socio-economic and cultural contexts, may face barriers in accessing healthcare, including financial constraints, social stigma, and limited decision-making power in health-related matters. In many societies, women are primarily responsible for managing the health of their children and families, which can lead to delays in seeking medical care for themselves. As a result, women may experience worse outcomes from these infections than men due to delayed diagnosis and treatment (Crump et al., 2023).
The burden of typhoid and Salmonella infections on women is further compounded by the lack of gender-sensitive healthcare policies and programs. While there has been progress in improving healthcare access for women, particularly through maternal and child health initiatives, there is still much to be done to ensure that women’s unique needs are met, especially in relation to infectious diseases like typhoid fever. Research has shown that women in rural areas are often excluded from public health interventions due to socio-cultural norms and gender roles, which restrict their ability to access health services in a timely manner (Yemata et al., 2021). These factors must be addressed to improve health outcomes for women and reduce the impact of typhoid and Salmonella infections.
In addition to the direct effects of the infections, typhoid fever and Salmonella infections have broader social and economic impacts on communities. In regions where these infections are prevalent, the loss of productivity due to illness and premature death can hinder economic growth. For women, in particular, the economic burden of illness is often more pronounced due to their roles as caregivers and breadwinners. Women who suffer from prolonged illness or who are unable to access treatment may experience increased poverty, further exacerbating social inequalities and hindering overall community development (Singh & Singh, 2021).
Addressing the challenges associated with typhoid fever and Salmonella infections in women requires a multi-faceted approach that includes improving access to healthcare, increasing awareness and education, promoting sanitation and hygiene practices, and strengthening disease surveillance systems. Public health campaigns that focus on improving water and sanitation infrastructure are crucial for preventing the spread of these diseases, particularly in rural areas (Buckle et al., 2012). Additionally, improving diagnostic capabilities and expanding access to effective treatment, including the availability of antibiotics that can combat resistant strains, are critical components of addressing the burden of these infections. It is equally important to integrate gender-sensitive policies into healthcare programs to ensure that women receive the care they need and that their specific healthcare needs are addressed in the context of infectious disease management.
1.2 AIM AND OBJECTIVE
The aim of this study is to explore the causes, effects, and challenges associated with typhoid and Salmonella infections in women, with a particular focus on how these infections impact their overall health, reproductive health, and access to healthcare services.
The specific objectives of the study are as follows:
- To identify the causes and primary modes of transmission of typhoid and Salmonella infections in women.
- To evaluate the health effects of typhoid and Salmonella infections on women, especially concerning reproductive health and maternal outcomes.
- To assess the challenges women face in accessing healthcare services for the prevention and treatment of these infections.
1.3 SIGNIFICANCE OF STUDY
This study is significant because it fills a crucial gap in the existing literature by specifically examining the impact of typhoid and Salmonella infections on women. While these infections are recognized as major public health concerns globally, there is limited research focusing on their particular effects on women, especially in resource-poor settings. Understanding the direct and indirect consequences of these infections on women’s health is essential for developing targeted public health interventions that address their unique health needs.
Women face distinct challenges when it comes to typhoid and Salmonella infections due to both biological and socio-economic factors. For instance, these infections can have serious reproductive health implications, particularly during pregnancy, and the social barriers women face—such as limited access to healthcare—further exacerbate their vulnerability. Addressing these issues requires gender-specific health strategies that prioritize women’s health and ensure that their needs are met in regions where these infections are highly prevalent.
The findings of this study will be valuable to healthcare providers, policymakers, and non-governmental organizations (NGOs) that work in the areas of infectious diseases, maternal health, and women’s health rights. The research will provide insights into the healthcare challenges women face, which can help in crafting policies and interventions aimed at improving access to both preventive and therapeutic healthcare services. Additionally, it will contribute to the broader conversation about gender disparities in healthcare access, emphasizing the importance of addressing these disparities through targeted, gender-sensitive policies.
By focusing on the specific health needs of women in relation to typhoid and Salmonella infections, this study aims to generate recommendations that can help reduce the burden of these diseases. These recommendations may include improving healthcare infrastructure, increasing awareness about the symptoms and treatment of these infections, and promoting hygiene and sanitation practices to prevent the spread of the bacteria. Ultimately, the goal of this research is to improve health outcomes for women in high-risk areas and to contribute to reducing gender inequalities in healthcare access.
1.4 SCOPE AND LIMITATION
This study focuses on typhoid and Salmonella infections among women, with an emphasis on the causes, health effects, and healthcare access challenges. It will examine these issues primarily in developing countries, where sanitation, water quality, and access to healthcare are significant concerns. The study will not explore the broader spectrum of foodborne diseases or other types of infections affecting women, but will concentrate specifically on Salmonella-related illnesses.
A limitation of the study is that it will rely on secondary data from published research, government health reports, and non-governmental organizations working in affected regions. Additionally, due to resource constraints, the study will not conduct primary data collection through field surveys or interviews. This may limit the study’s ability to capture firsthand accounts of the challenges faced by women in accessing healthcare for these infections.
1.5 STATEMENT OF THE PROBLEM
The problem this study addresses is the significant gap in research concerning the specific impact of typhoid and Salmonella infections on women, especially in regions with limited healthcare access. While Salmonella infections, including typhoid fever, have been widely studied in general populations, much of the literature does not focus on the particular challenges women face in relation to these diseases. Typhoid fever, caused by Salmonella enterica serotype Typhi, remains a major health issue in many developing countries, where sanitation issues and limited access to medical care exacerbate the spread of the bacteria (Stanaway et al., 2023). Women in these areas are at greater risk due to their biological vulnerability, including complications during pregnancy and menstruation, as well as additional socio-economic barriers that hinder their access to effective healthcare (Crump et al., 2023).
A key gap in existing research is the limited exploration of the gendered aspects of Salmonella infections. While studies such as those by Deksissa and Gebremedhin (2019) have highlighted the general epidemiology of Salmonella infections, there is a scarcity of research addressing how these infections uniquely impact women. Pregnancy and postnatal care add layers of complexity to the management of typhoid and Salmonella infections in women, but studies that focus specifically on maternal health in relation to these infections are lacking (Haftu et al., 2024). Furthermore, social and cultural norms often restrict women’s ability to seek timely and adequate medical care, with financial constraints and gender inequality compounding the problem (Ajibola et al., 2024).
Another critical gap is the insufficient attention given to the socio-economic factors influencing women’s healthcare access for the prevention, diagnosis, and treatment of Salmonella infections. In low-income settings, women are disproportionately affected by inadequate healthcare resources, which delays diagnosis and increases the risk of complications (Naheed et al., 2022). Many studies have documented the burden of Salmonella infections (Buckle et al., 2012) but fail to account for how these infections specifically affect women’s health outcomes in high-risk regions.
Additionally, the increasing problem of antibiotic resistance in Salmonella strains is underexplored in the context of women’s health. The persistence of multi-drug resistant strains poses unique challenges to women, particularly in regions where healthcare infrastructure is underdeveloped (Ohanu et al., 2019). Understanding how these resistant strains affect women differently is vital for informing treatment strategies and public health responses.
By addressing these gaps, this study seeks to enhance understanding of the unique health challenges women face regarding typhoid and Salmonella infections and provide actionable recommendations for improving healthcare access and outcomes in women, particularly in regions with high infection rates. This research will contribute valuable insights to the development of gender-sensitive public health policies and interventions, aimed at reducing the burden of these infections on women (Prasad et al., 2024).
1.6 DEFINITION OF TERMS
Typhoid Fever: A systemic illness caused by Salmonella enterica serotype Typhi, characterized by high fever, weakness, abdominal pain, and sometimes gastrointestinal symptoms, transmitted mainly through contaminated food and water.
Salmonella Infections: A group of bacterial infections caused by various serotypes of the Salmonella genus, leading to gastroenteritis or systemic infection, often spread through contaminated food products like meat and eggs.
Reproductive Health: A state of physical, mental, and social well-being in all matters related to the reproductive system, which includes safe pregnancy, childbirth, and the access to family planning and sexual health services.
Healthcare Access: The ability of individuals to obtain needed healthcare services, which is often influenced by factors such as income, geography, education, and gender.
Gender-based Inequality: Disparities between men and women in terms of access to resources, healthcare, education, and employment opportunities, often exacerbating health outcomes in women.
